When asked to comment about tissue plasminogen activator and stroke, I thought much about my own practice and role in stroke care. The reanalysis by Hoffman and Schriger1 of the original tissue plasminogen activator trial funded by the National Institute of Neurological Disorders and Stroke (NINDS)2 in this issue will likely draw criticism from the proponents of this therapy. For the benefit of the patient and the practicing emergency physician, I believe we need to declare a moratorium on the efforts to make this therapy the expected “standard of care” in community hospitals
